Einführung

Acidity (hyperacidity or acid dyspepsia) is one of the most common gastrointestinal disorders. It is caused by excess secretion of hydrochloric acid (HCl) from the gastric glands of the stomach. Gastric acid is normally required to digest ingested food particles and maintain the microbial balance. But when there is too much acid or the defence mechanisms of the mucosa are weakened the gastric juices irritate the lining of the stomach and oesophagus, leading to severe burning behind the breastbone and indigestion.
This condition is classically referred to as Amlapitta in Ayurveda. The term is a compound of Amla (sour taste) and Pitta (one among 3 Doshas). In this blog, let’s explore the causes, symptoms, diagnosis, treatment and home remedies for acidity.

  • Stomach contents flowing back into the food pipe, often because the valve between them doesn’t prevent reflux well.
  • Being overweight, smoking, alcohol use, some eating habits, and certain medicines, which are associated with reflux.
  • Pregnancy and hiatus hernia, which can make reflux more likely.
  • Heartburn related to food ingestion
  • Stress and increased cortisol levels

Ayurveda classifies the etiopathological triggers (Nidana) of acidity under four broad domains:

Aharaja Hetu – Dietary Factors

  • Heavy (Guru), oily (Snigdha), sour (Amla), salty (Lavana), spicy (Katu) and fatty foods.
  • Incompatible food combinations, eating before the previous meal is digested, eating at irregular intervals and overeating.
  • Processed junk foods, caffeine, carbonated drinks, alcohol, citrus fruits and fermented products overstimulate acid secretion.

Factors of the Lifestyle – Viharaja Hetu

  • Sleeping in the daytime immediately after meals and disturbed sleep or keeping awake at night for long disrupt biological rhythms.
  • Suppressing the natural urges of the body like hunger, sleep or the urge to defecate hinders the motility of digestion.

Psychological Factors – Manasika Hetu
Chronic emotional stress is exemplified by the classic triad of “hurry, worry and curry”. Psychological stress activates the Hypothalamic-Pituitary-Adrenal (HPA) axis, leading to increased cortisol and catecholamines.

  • Anger, anxiety and grief are some of the emotions that negatively affect the mind and digestive system, leading to irregular digestion.


External and Systemic Causes – Agantuja Hetu

  • Regular use of Non-Steroidal Anti-Inflammatory Drugs (NSAIDs), aspirin, corticosteroids and antibiotics causes irritation of the gastric mucosa.
  • Obesity, pregnancy, hiatal hernia, H. pylori infection and smoking are examples of things that relax the lower oesophageal sphincter.

What Are the Symptoms of Acidity?

The clinical manifestations of acidity vary from mild retrosternal discomfort to systemic distress:

  • Hritkantha Daha (Heartburn): Burning sensation of severe type from the epigastrium upwards to the chest and throat.
  • Acid Eructation: Reflux of sour or bitter fluids in the throat and mouth.
  • Indigestion & Nausea: Delay in digestion of even light food with constant nausea.
  • Epigastric Heaviness & Fatigue: Stomach heaviness and fatigue without effort.
  • Loss of Appetite: Lack of appetite due to impaired Agni.

Classical texts categorise Amlapitta into two major directional subtypes:
1. Urdhwaga Amlapitta (Upward Direction): Here the aggravated Pitta rises upwards into the chest, throat and head, giving rise to sour belching, heartburn, nausea, vomiting of sour fluids and headache.
2. Adhoga Amlapitta (Downward Direction): Aggravated Pitta moves downwards to the lower GI tract with abdominal burning, excessive thirst, fainting and loose, foul-smelling stools.

Acidity diagnosis in Ayurveda requires assessing the individual patient holistically. The physician considers symptoms, diet, lifestyle, stress levels, sleep pattern, bowel movements and other aspects of the patient’s health, in addition to the dosha balance. Some other diagnostic techniques include Ayurveda diagnostic techniques, such as Nadi Pariksha (Pulse Assessment) and Jihva Pariksha (Tongue Examination).

Medical and symptom history form the preliminary step for diagnosing acidity. Other tests may be done based on the symptoms’ nature, severity, and cause.

The following diagnostic tests can be done for diagnosing acidity:

  • Personal and medical history: Involves the history of symptoms, dietary patterns, lifestyle, medication history, bowel movements, stress and any previous problems with the digestive system.
  • Endoskopie des oberen Gastrointestinaltrakts: A thin tube with a camera is passed via the mouth and oesophagus to the stomach to look for any signs of inflammation and ulcers.
  • Ösophagus-pH-Überwachung: This test is used for measuring the amount of acid in the oesophagus.
  • Ösophagusmanometrie: Assesses the pressure and movement of the oesophagus and may be advised when swallowing or motility problems are suspected.

Differentialdiagnose

It is important to distinguish acidity from other GI and systemic entities:

  • Gastroösophageale Refluxkrankheit (GERD): Acid reflux leading to erosion of mucosa; comparable to Urdhwaga Amlapitta.
  • Gastritis & Peptic Ulcers: Severe localised abdominal pain related to food intake; pain occurs during digestion, while pain can persist regardless of food intake.
  • Dyspepsie: indigestion, acid belching and dizziness due to acidified chyle without chronic mucosal involvement.
  • Intestinal Disorders: Frequent loose motions mixed with unformed chyle, acid eructation, thirst, small intestinal dysfunction.

Ayurveda treatment for Acidity

Ayurveda management aims to treat the root causes with a structured three-pronged protocol:

Nidana Parivarjana (Removal of Causative Factors)
One of the most important parts of treatment is strict withdrawal from dietary, lifestyle and mental triggers.

Shodhana Chikitsa (Purification & Detoxification Therapy)
In case of severe aggravation of Dosha, purification is indicated:

  • Therapeutic Purgation (Virechana Karma): The main line of therapy in acidity. It removes aggravated Pitta and Kapha from the root and reduces the sour eructation, etc. symptoms.
  • Therapeutic Emesis (Vamana Karma): Indicated in acidity with predominance of congestion and nausea.

Shamana Chikitsa (Palliative Therapy)

Classical herbal and herbomineral formulations are used to pacify the excess heat and soothe the gastric mucosa. They also aid in improving digestion
After meals, one can practise vajrasana and pawanmuktasana. These asanas improve the circulation in the abdomen. Cooling pranayama techniques such as Sheetali and Anuloma Viloma are known to calm the autonomic nervous system and regulate acid secretion.

Simple dietary measures can provide effective relief for mild, acute symptoms:

  • Tender Coconut Water: It calms the stomach lining and balances the gastric pH.
  • Fennel seeds: Chewing half a teaspoon of fennel seeds after meals helps relieve bloating and acid reflux.
  • Coriander Seed Infusion: Soak the crushed coriander seeds in water overnight and drink the infusion to cool the body and pacify the internal heat.
  • Rose Petal Jam (Gulkand), Indian gooseberry (Amalaki): Consuming these helps to alleviate burning sensation in the stomach, reduce sour belching and soothe Pitta.

Conventional antacids are useful to get temporary relief from the symptoms. Ayurveda gives a root-cause solution through Shodhana, soothing herbal remedies (Shamana) and disciplined dietary habits. By incorporating these measures, one can get rid of acidity and balance the digestive system dysfunction.
