Treatment Procedure

1. Poorva Karma (Preparatory Procedures)

Initial procedures to prepare the body for intense Vamana. It aids in mobilising and dislodging the accumulated toxins and vitiated Doshas from the body’s deeper tissues, making them easier to eliminate during the Pradhanakarma (main procedures). Poorvakarma is attained through the following steps:

  • Deepana–Pachana: Administration of digestive herbs to improve appetite and digest undigested matter (3-7 days).
  • Takrapana: Administration of medicated buttermilk to open up body channels and to improve the absorption of the medicine. Due to its Rooksha (dry), Ushna (hot), Amla-Kashaya Rasa (sour and astringent taste), and Ushna Virya (hot potency), Takra helps in Rookshana (drying the body), Srotoshodhana (cleansing the body channels), and Deepana (stimulating digestion), which helps to decide the right dose of medicine for proper Shodhana (cleansing).
  • Abhyantara Snehana: Internal administration of medicated Sneha, usually in the form of ghee or oil, depending on the disease condition, which helps to soften and loosen the morbid Doshas, making it easier to extract them from their deeper tissues. It also improves Jatharagni (digestive capacity), supporting better absorption, assimilation, and metabolism. The Sneha is given in gradually increasing doses over 3 to 7 days, depending on the individual’s Agnibala (digestive strength), Koshta (gut behaviour and bowel patterns), and overall response to the medicine, until the person shows signs of proper Snehana.
  • Bahya Sneha – Swedana: In the form of Abhyangam (application of medicated oil over the whole body) followed by Swedana (steam therapy). This process helps in Doshoklesha (excitation and mobilisation of the Doshas), promotes the liquefaction of accumulated Doshas and dilates the Srotasas (body channels), thereby allowing the Doshas to move from the Shakha (deep-seated tissues) to the Koshta (gastrointestinal tract). This prepares the body for easier and more effective elimination.
  • Diet: An easily digestible, warm, and liquid diet that is non-obstructing, neither heavy nor very oily/sticky, and not complex, in limited quantity, is to be administered. On the day before Vamana, food containing dahi (Curd), tila (Sesame seeds), urad dal, milk sweets, etc., is to be given to excite the Doshas for better results. 

2. Pradhanakarma (Main Procedure)

Pradhanakarma is done through the following steps: 

  • Counselling the patient for a better understanding of the process and its seamless execution to yield the most favourable and promising outcomes. After being explained the entire procedure, well-framed informed consent is obtained from the patient
  • During early morning, around 4:00 am to 7:00  am, the patient who has slept well, has had his previous day’s meal properly digested and has had his bowel and bladder cleared completely, is given an oil application on the body and a steam procedure. Patients are also advised to wear loose-fitting and comfortable clothing during the procedure.
  • Basic vital parameters like BP, Pulse rate, FBS, Temperature, Weight, etc., are monitored just before the procedure.
  • Initially, the patient is administered milk or sugarcane juice to fill the stomach in order to prevent throat irritation and mucosal erosion during vomiting and also to ease the process. After this, the patient is given emetic herbal preparations according to the condition, age and strength of the patient, under close supervision.
  • The onset of nausea, retching, and vomiting is carefully monitored. The patient is instructed to allow the vomiting reflux to get completely evident and not to exert too much to expel the vomitus quickly.
  • If the Vegas (vomiting bouts) do not begin on their own, supportive methods can be used, such as giving water boiled with herbs like Pippali (long pepper), Amalaki (Indian gooseberry), Sarsapa (mustard), and Lavana (salt), or by gently stimulating the pharynx using a soft rubber catheter or with fingers of the patient itself without causing any injury to the throat. During the procedure, liquids prepared with Yashtimadhu (licorice) or Saindhava (rock salt) may be administered repeatedly. 
  • To judge effectiveness, the quality and quantity of vomitus are observed for the contents, consistency, colour, and odour.
  • Throughout the process, hydration and comfort are maintained.
  • Samyak Lakshana (Signs of Proper Vamana): A well-conducted Vamana has several clear indicators:

Effortless vomiting: The vomiting occurs smoothly, expelling Kapha without strain or force.

Sequential expulsion of Doshas: The expelled material initially consists predominantly of Kapha dosha, followed in succession by Pitta and finally Vata.

Lightness in the body: The individual feels physically light and mentally clear after the expulsion.

Improved digestive fire: There’s an improvement in appetite and digestive capacity following the procedure.

3. Paschat Karma (Post-Vamana Care)

Post-treatment, the focus is on proper nutrition and ample rest to restore and strengthen digestion and physical strength.

  • Easily digestible foods are given in the diet (e.g., rice gruel, vegetable soups) along with sufficient electrolytes and fluid supplementation to compensate for its loss. The patient gradually returns to a regular diet.
  • Monitoring for adverse effects such as dehydration or electrolyte imbalance.
  • Follow-up visits to ensure proper recovery.
